Excited to share the launch of IMMERSE+ (Cardio-Plus) a cutting-edge project transforming care for patients with multimorbidities—particularly those living with cardiovascular disease and mental health challenges.
IMMERSE+ integrates Experience Sampling Methodology (ESM) into a Digital Mobile Health (DMH) tool, enhanced with machine learning, to enable truly patient-centered care. This approach empowers patients to actively participate in their treatment, improving adherence, self-management, and overall well-being.
The project also explores the feasibility of integrating DMH into routine clinical practice, identifying key barriers and facilitators in collaboration with patients, healthcare providers, and policymakers.
Funded by the EU NextGenerationEU Recovery and Resilience Plan for Slovakia under the project No. 09I01-03-V04-00101. IMMERSE+ is a matching grant to the EU H2020 project IMMERSE.
